Under-eye hollowing: causes and cosmetic choices

A shadow or hollow beneath the eye can be related to facial anatomy and changes in the surrounding tissues over time. It is not the same as a fine surface line or swelling. Lighting and skin tone can also affect how the area looks. This guide discusses a cosmetic concern; it cannot identify the cause of a new symptom from a description alone.

What should be assessed?

Point to the exact area you mean and describe when you first noticed the change. A clinician can consider the contour, skin, previous procedures, relevant health information and the appearance you hope to change. Is an injectable always the answer?

No. An assessment may find that a procedure is unsuitable or unlikely to give the change you want. If a filler or another injectable is considered, ask for the exact product, intended area, practitioner, realistic limits, aftercare and alternatives including no procedure. Filler does not correct every shadow or reliably create a rested appearance. The visible effect and recovery vary; no standard amount or repeat interval fits everyone.

What risks and symptoms matter?

The area near the eye needs particular care. Bruising, swelling, tenderness, asymmetry, infection and nodules are possible with an injection. Unintended injection into a blood vessel can cause serious injury, including visual harm. Seek urgent assessment for severe or increasing pain, pale or dusky skin, or visual symptoms during or after filler. New, changing, painful or one-sided swelling, visual symptoms or another unexplained change needs appropriate healthcare assessment rather than a cosmetic booking.
